Financial Arrangements

In orthodontic cases, a designated fee is paid depending on the severity of the problem for the appliances and the services of the doctor. An additional fee may be incurred if treatment is extended.

The initial payment is due at the placement of the appliances. Monthly payments are due on the first of the month. Payments will continue in the event of an extended absence, vacation, or missed appointments.

The estimated treatment time is based on professional experience and averages and requires total compliance from the patient. If treatment is extended due to poor treatment response, poor cooperation or an unexpected change in treatment plan, additional fees may apply. If a patient relocates (transfers treatment) to another area, requiring treatment to be completed by another orthodontic practice, our total fee will be adjusted to reflect the services we have provided.

We accept assignment from most insurance plans. We will gladly submit the necessary information to your dental insurance carrier.

Fees

Orthodontic treatment is an excellent investment in the overall dental, medical and psychological well being of children and adults. Financial considerations should not be an obstacle to obtain this important health service. Being sensitive to the fact that all people have different needs in meeting their financial obligations, we provide several different payment options.

Option A:
Plan #1: Payment in full with a 5% bookkeeping courtesy.
Plan #2: An initial payment (to fit your individual needs) with monthly payments extended over a period of months.

There is no interest or finance charge associated with our in-office payment plans, provided that the schedule of payments is kept current. Monthly payments represent a convient method of payment and are not based on number of appointments per month, year or length of treatment. Actual treatment time may be shorter than your payment period. Your total contract balance is due at the time the appliances are removed.

Option B:
OFP-Orthodontists Fee Plan: No initial payment. Take up to 60 months to pay with low monthly payments, which includes a minimal finance charge. For your convenience, we can automatically bill your credit card company (Visa, Mastercard, and Discover are accepted).

Insurance

We are happy to cooperate with you and your insurance carrier to help you receive the maximum benefits available under your policy. We will accept assignment on covered expenses. However, your insurance policy is a contract between you and your insurance carrier. We will answer any questions to the best of our knowledge about your policy, but ask that you address any questions that we cannot answer with your employer or insurance carrier. Although you have dental insurance with orthodontic coverage, you are ultimately responsible for the account.

Please let us know if you change insurance carriers/employment during treatment – this may change your benefits. If you have primary and secondary insurance, you may have a clause in your policy prohibiting duplication of benefits.
